This series of illustrations depicts multiple lumbar herniations at L2-3, L3-4 and L4-5, facet hypertrophy with foraminal stenosis. Discectomies are made at L3-4 and L4-5 levels with double peek implants are placed within the interspaces. Pedicle screws are placed bilaterally at L3, L4 and L5 and connected with rods.
